Key Policies & Resident Rights
These policies are provided in full in our Residency Agreement. Below is a plain-language summary of the areas families ask about most.
Every resident of Serenity Plus AFH has rights protected by Washington State law (RCW 70.128 and WAC 388-76) and by our own internal standards. These include:
- The right to be treated with dignity and respect at all times
- The right to privacy in their room, personal communications, and personal care
- The right to participate in decisions about their own care and treatment
- The right to refuse treatment or medication (with documented informed consent)
- The right to have their personal property respected and protected
- The right to file a complaint or grievance without fear of retaliation
- The right to receive visitors during posted visiting hours
- The right to manage their own finances or designate a representative to do so
- The right to be informed of changes to their care plan in advance
A full copy of the Washington State Adult Family Home Resident Rights is provided to every resident and their representative upon admission.
Serenity Plus AFH accepts residents based on our ability to safely meet their care needs within our licensed scope of practice. Our admissions process includes:
- A pre-admission assessment to evaluate the resident's medical, functional, and behavioral needs
- Review of physician orders, medication lists, and current care documentation
- A home visit or tour for the prospective resident and family
- A written Residency Agreement reviewed and signed prior to move-in
- A 30-day trial period during which either party may terminate the agreement with appropriate notice
We do not discriminate in admissions on the basis of race, ethnicity, religion, gender, sexual orientation, disability, national origin, or source of payment (within licensed scope).
We are transparent about conditions we cannot safely manage. If a resident's needs exceed our scope, we will say so clearly and assist in identifying appropriate alternatives.
Our monthly care fee is established in the Residency Agreement and covers the following included services:
- Room and board (private or semi-private)
- Three home-cooked meals and two snacks per day
- 24/7 caregiver supervision and personal care assistance
- Medication administration and management
- Laundry and housekeeping
- All scheduled and recreational activities
Services not included in the base rate (and billed separately or by external providers) include: physician visits, specialist care, pharmacy costs, durable medical equipment, and ancillary therapies.
Rate increases require 30 days written notice. Fee adjustments due to significant changes in care level are discussed with the family before implementation. We do not charge hidden fees — every cost is itemized in the Residency Agreement.
We accept private pay. Families are encouraged to consult with a financial advisor regarding long-term care insurance, veterans benefits (VA Aid & Attendance), and other funding options that may apply.
Family and friends are welcome to visit residents during the following hours:
- Visiting hours: 9:00 AM – 7:00 PM, seven days per week
- Visits outside these hours may be arranged in advance with the care team
- Hospice residents: extended and overnight family visits are welcomed and facilitated
Visitors are asked to sign in upon arrival, respect other residents' privacy, and inform the care team of any concerns observed during their visit. All visitors must be free of communicable illness.
All medications — prescription and over-the-counter — are managed by our caregivers in accordance with current physician orders. Our medication policy includes:
- All medications stored in a locked cabinet inaccessible to residents and visitors
- Medications administered only as prescribed — no deviation without physician authorization
- Detailed medication administration records (MARs) maintained and available to authorized parties
- Pharmacy coordination handled by our team — families notified before any refill is needed
- Any medication error is documented, reported to the prescriber, and disclosed to the family
- Controlled substances managed in compliance with Washington State law
Residents and families have the right to raise concerns, file complaints, and expect them to be taken seriously and addressed promptly. Our grievance process:
- Any concern may be raised verbally or in writing to the administrator at any time
- Written grievances receive a written response within 5 business days
- Grievances are never held against the resident in any form
- If a concern is not resolved to the family's satisfaction, they may contact Washington State DSHS Adult Family Home licensing office directly
- The DSHS complaint line is: 1-800-562-6078
We actively encourage feedback. A culture of accountability begins with the willingness to hear when something isn't right.
A resident may be discharged only for reasons permitted under Washington State law and our Residency Agreement. Permissible reasons include:
- The resident's care needs exceed what we can safely provide
- Non-payment of fees following written notice and cure period
- Behavior that poses a documented threat to other residents or staff
- Facility closure
We are required to provide a minimum of 30 days written notice in most circumstances. In the event of an involuntary discharge, we assist families in identifying alternative placements and provide all necessary care documentation to the receiving provider.
Voluntary discharge by the family or resident requires notice as specified in the Residency Agreement. We work cooperatively with families to ensure safe and smooth transitions.
This page provides a summary of key policies for general reference. The full Residency Agreement — which governs the legal relationship between Serenity Plus AFH and each resident/representative — is provided in complete form prior to admission.
