What Medical Management Means at Serenity Plus
In a large facility, medication management often means a nurse cart rolling down a hallway twice a day. At Serenity Plus AFH, it means a trained caregiver who knows every resident's regimen, tracks every dosage, and communicates directly with physicians — because with only six residents, there is no room for anything to be routine or impersonal.
We handle the full complexity of each resident's medical picture, coordinating between primary care doctors, specialists, pharmacies, and visiting nurses so that families don't have to.

"Our caregiver-to-resident ratio means your loved one's health status is known in detail — not summarized in a shift note that nobody reads."
Our Approach to Health Monitoring
We believe that effective medical management begins with observation. Because our caregivers spend extended time with each resident every single day, they notice changes that a rotating staff member would miss — subtle shifts in appetite, mood, cognition, or gait that can be early indicators of a developing health issue.
Any concern is documented, escalated to the appropriate clinical contact, and communicated to the family without delay. We don't wait for a scheduled care conference. We call.
Daily Monitoring
Vitals, behavior, appetite, and sleep are observed and documented every day for every resident.
Immediate Escalation
Any deviation from baseline triggers immediate review — by caregiver, then supervisor, then physician as warranted.
Family Transparency
Families receive prompt notification of any health change, not a summary at the end of the month.
Medication Safety
Medications are stored securely and dispensed precisely. No self-administration by residents with cognitive impairment.
Coordination with External Care Teams
Serenity Plus AFH operates as a collaborative partner in each resident's broader care network. We work directly with:
- Primary care physicians and geriatric specialists
- Home health agencies for skilled nursing visits
- Physical, occupational, and speech therapists
- Hospice organizations when applicable
- Mental health providers and psychiatric nurses
- Local pharmacies and compounding specialists
